Community Partners Portal

The Community Partners (CP) Program Portal is for the submission of member enrollment and disenrollment requests for the CP Program.

Behavioral Health (BH) and Long-Term Services and Supports (LTSS) CPs are community-based entities that work with Accountable Care Organizations (ACOs) and Managed Care Organizations (MCOs) to provide care coordination supports to certain members identified by ACOs, MCOs, and Department of Mental Health (DMH).

Only identified users from ACOs, MCOs, BH CPs, and LTSS CPs are authorized to access this portal to submit CP Program enrollment and disenrollment requests.
